No insurance? That is not a problem. We offer affordable self-pay options, and Dr. Juarez will always give you a straight answer about what your care costs and what it takes, so there are no surprises.

Hurt in a car accident? We treat the injury and document everything your insurance or attorney needs for your claim, so you can focus on healing instead of paperwork. The same goes for an injury on the job.

What to Bring to Your First Visit

  • A photo ID
  • Your insurance card, if you have one
  • Your completed new patient form, or arrive a few minutes early to fill it out
  • Any recent X-rays, MRI, or imaging reports you have
  • Accident or claim paperwork, if you were hurt in a car accident or at work
